Please try Head and Shoulders Bare. If your scalp is painful, have you asked your dermatologist about possibly prescribing a Prednisone 10 day taper to quiet your immune system and allow it to regroup? Have you tried Derma Smooth, a prescription oil for the scalp? I apply it at bedtime every night: an ounce of prevention is worth a pound of cure! ( I think , just saying). I also use Clobetasol ointment 0.05%). When a spot on my arm, ankle, etc. I apply it to head the resultant nodule/lesion off before the eruption.

Norma, I wish I had some good advice to offer to you as I have definitely been a real "Sore Head" for 15 years and have tried anything and everything to possibly relieve the horrific pain, itch-scratch cycle, burning, stinging, bleeding, scabs, with Lichen Simplex Chronicus (LSC), but nothing works for me. Even after 8 Dermas, & 1 Allergist they were of no help! I am definitely at my wits end, & just anxiously waiting for the FDA to approve the new drug, Nemolizumab which is specifically for this rare, no cure disease for some much-needed relief hopefully! I am on antianxiety, antidepression meds, plus CBD which helps somewhat as I have 15-20 nodules (flat type which are encased by LSC), & decapitation comes to my mind a lot! 😱

Hi Norma, I'm sorry to hear about the pain you're experiencing. Here are some steps you can take to manage prurigo nodularis (PN) on your scalp:

1. Avoid Scratching: Scratching can worsen the itchiness and lead to more nodules and potential infections. Consider using a lightweight, breathable head covering to minimize direct scratching.

2. Topical Treatments: Applying medications to the scalp can be tricky. The American Academy of Dermatology Association (AAD) suggests parting your hair in sections and applying the medication directly to the skin. Use a dropper for liquid medications or your fingers for creams and foams.

3. Shampoos: Use over-the-counter shampoos with a pH between 4.5 and 6.0 to help reduce itchiness.

4. Cold Compression: Applying an ice pack or a cold, wet cloth to the affected area for 5-10 minutes can help reduce itching.

5. Avoid Triggers: Identify and avoid triggers such as heat, humidity, stress, dry air, and irritating fabrics or hair care products.

6. Consult Your Dermatologist: Since you've already seen a dermatologist, it might be helpful to follow up with them if your current treatments aren't effective. They might recommend treatments like corticosteroids, immunosuppressants, antihistamines, or phototherapy.

If your symptoms persist or worsen, consider seeking a second opinion from another specialist.
